Paget's Disease CT Findings

  • Lytic:
    • Osteolysis with resorption
    • Osteolysis has sharp, leading edge (“blade of grass” or “candle flame”)
  • Mixed:
    • Cortical thickening
    • Osseous expansion
    • Coarsening and thickening of trabeculae
  • Sclerotic:
    • Osteosclerosis
    • Fatigue fractures and bowing deformities
  • Usually involves multiple bones

Etiology:

  • Unknown
  • Familial history

Epidemiology:

  • Typically presents over age 50
  • More common in people of northern European heritage
  • Slightly more common in males

Presentation:

  • Typically asymptomatic
  • Bone or joint pain
  • Bowing of the long bones
  • Bones that fracture easily

Prognosis:

  • There is no cure for Paget’s disease but it can be managed
