Diagnosis

Jugular Foramen Schwannoma, right

Note
42-year-old male with right-sided pulsatile tinnitus. Centered in the right jugular foramen and extending into the upper aspect of the carotid sheath on the right, there is a T1-isonintense, T2/FLAIR-isointense mass which mildly enhances. The findings are most compatible with a right jugular foramen schwannoma. Jugular foramen schwannoma are benign tumors of differentiated Schwann cells involving the hypoglossal, vagus, or accessory cranial nerves. Typical age at presentation is 45. Sensorineural hearing loss is present in 90%. Complete surgical removal is the goal.
