Seizures

Note
This is a case of generalized tonic-clonic seizures in a 52-year-old female. Images 1 and 2 demonstrate abnormal T2/FLAIR hyperintense signal involving the cortical grey matter of the bilateral parieto-occipital regions. There is associated gyral swelling and sulcal effacement without midline shift. The subcortical white matter is characteristically spared. Images 3 and 4 demonstrate a peripheral wedge-shaped region of cytotoxic edema in the right occipitotemporal region. There is corresponding decreased perfusion on the mean transit time and cerebral blood volume images. This patient demonstrates imaging features which can be seen in the generalized convulsive seizures.
