CASE NUMBER 104
Diagnosis

Segond Fracture

Note
This is a case of a 13 year old girl who presented with sudden onset knee pain while playing soccer. A frontal radiograph of the knee demonstrates an avulsion fracture of the lateral tibial plateau. This fracture occurs secondary to avulsion of the lateral tibia plateau at the site of attachment of the lateral capsular ligaments. This fracture pattern is critical to recognize as it is highly associated with tear of the anterior cruciate ligament. An additional radiographic sign which can be seen with an ACL tear is a deep lateral sulcus sign which is secondary to an impaction fracture of the anterolateral lateral femoral condyle which occurs with full thickness anterior cruciate ligament rupture.
