Sequestered Disc

Note
48-year-old male with chronic back pain unrelieved with conservative management. There is minimal loss of disc height at the L4-L5 and L5-S1 disc levels. There is a small disc herniation at the L4-L5 disc level with disc degeneration at the L4-L5 and L5-S1 disc levels. An ovoid T1- and T2/STIR hyperintense lesion in the right paracentral region and extending towards the subarticular zone is demonstrated. It contacts and minimally displaces the right descending L5 nerve root. The lesion does not enhance on the post contrast sequence. The findings are most compatible with a sequestered disc fragment. The differential includes sequestered disc fragment, neurogenic tumor, or synovial cyst. Disc sequestration is used to describe displaced disc material that has completely lost continuity with the native disc.
