Diagnosis

Meningitis.

Note
There is extensive lack of FLAIR suppression in the cerebral sulci predominantly affecting the frontal and parietal lobes near the vertex. There is corresponding leptomeningeal enhancement, as well as pachymeningeal enhancement. This patient had a known established diagnosis of pneumococcal meningitis. On diffusion-weighted imaging, restricted diffusion developed between examinations in the right frontal lobe. This acute ischemia was likely secondary to meningitis-associated vasculitis.
