Mazabraud Syndrome

56-year-old female with chronic right hip pain. There are two circumscribed T1-hypointense, STIR-hyperintense lesions in the right hemipelvis. The first lesion is centered within the right iliac bone superior to the acetabulum with well circumscribed margins, no diffusion restriction, and minimal peripheral nodular enhancement. The second well circumscribed lesion is centered within the right gluteus medius muscle belly with circumscribed margins, no diffusion restriction, and minimal heterogeneous enhancement. Imaging findings within the right iliac bone are compatible with cystic fibrous dysplasia while those in the right gluteus medius muscle are compatible with a soft tissue myxoma. This is a patient with a known history of Mazabraud syndrome which consists of fibrous dysplasia with multiple soft tissue myxomas. Mazabraud syndrome is more frequent in women and usually presents in middle age. There is increased risk of osseous malignant transformation.
