    Abstract

This report examines why health systems remain early in their agentic artificial intelligence (AI) journey despite growing interest and clear long-term potential. Drawing on survey data from 30 health system leaders and case studies from the AI Collaborative, it identifies the core barriers that keep organizations from moving beyond pilots into enterprise-level deployment. Findings highlight an interconnected readiness gap across governance, data infrastructure, and workforce capability that limits scalable adoption. The analysis outlines the foundational steps health systems must take to bridge this gap and position for agentic AI transformation over the next 35 years.
