I am trying to find a template for documenting our monthly radiation log?  If anyone has suggestions that would be great.

Yes, we are doing a monthly dose log?  As of right now we just pick 10 patients and record their CDTI number without the name and any identifiers.  I was just wondering how others were documenting this.

not sure how this helps exactly unless you follow specific study types with the info as well

We have to pick abdomens, heads and pediactrics.  The thing is we don't do many heads, no pediatrics and actually no abdomens only, just abdomen/pelvis.  It doesn't seem to be a real fool proof system.  This is why I was wondering if other places were recording radiation patient doses.
