I’m looking into the necessity and legality of patient history / screening forms. My facility fills out a lengthy patient history / screening form for each patient who receives IV contrast. Is a patient history form necessary? If not, does simply having the Technologists ask about allergies, diabetes, and pregnancy before administering contrast suffice?

even thought we have a history form it is best to re-ask the same questions. sometimes patients check the wrong box so this is added safety
